Dean Individual Health Plans
Upfront benefits and copays are the backbone of these plans, including the Dean Copay with no deductible, and two deductible plans with unlimited office visits and urgent care for $30. Once you have met the deductible your plan covers 100 percent of any covered services that do not have a copay. Emergency care, inpatient and outpatient hospital services, and mental health care are also covered for a fixed copayment, making it easy and affordable to obtain the services you need.
With a range of deductibles to choose from, these plans include preventive care and generic prescriptions as soon as you enroll. Additional prescriptions are available for a copay when you pay a $250 pharmacy deductible. Coverage for office visits, hospital care, mental health care, and emergencies is 80/20 after deductible, unless you choose the $5000 copay plan, which covers all services in full. These plans provide a low monthly rate on access to the award-winning Dean HMO network.
Four deductible choices provide a unique combination of HMO coverage with health savings account compatibility. Federally qualified for used with an HSA, the Dean HSA plans cover 80 percent of all services, with the exception of the $5000 deductible plan with 100 percent coverage. Preventive care is available for free as soon as your plan starts, and you have access to $10 generic prescriptions and other tiers with an optional rider. Pay for emergency room visits, doctor’s office services, and hospital care with your HSA until your deductible is met, and receive great coverage afterwards.
