Care Select

Care Select is an Indiana Medicaid program for individuals who may have special health needs or benefit from specialized attention. As with other Medicaid programs, Care Select requires members to select a primary care physician to orchestrate their care as part of one of the Care Management Organizations (CMOs) contracted with the state. Your individual needs and preferences for care and benefits will be addressed by your CMO in order to provide the most helpful and successful health care services possible.

Persons eligible for the Care Select program may be aged, blind, disabled, wards of the court, foster youth, or children receiving adoptive services. In addition to these qualities, a qualifying applicant must also have one of the following medical conditions:

  • Asthma
  • Congestive Heart Failure
  • Depression
  • Diabetes
  • Heart Failure
  • Hypertensive Heart Disease
  • Hypertensive Kidney Disease
  • Rheumatic Heart Illness
  • Serious Emotional Disturbance (SED) in wards and fosters
  • Severe Mental Illness

While Care Select is beneficial for those who do meet the criteria, it is entirely optional. If you feel you should be in this program, discuss the possibility with your doctor. Your doctor can request your inclusion in this program if you have one of the listed conditions and meet the other criteria. Those who qualify for Care Select and do not want to participate can rely on Traditional Medicaid for coverage.



Care Select Benefits

Each health care provider offers a different set of benefits, though they generally cover the same types of services. Care Select CMOs include the following groups of medical care services:

Preventive Care

Services to help you stay healthy, such as:

  • Disease management
  • Family planning
  • Prescription medications
  • Physician’s office visits

Medical Transportation

Up to 20 one-way trips per year without pre-authorization.

Mental & Behavioral Health Care

Coverage for outpatient mental health services from:

  • Physicians
  • Local mental health centers
  • Social workers in your community

Disease Management

Services to educate and give members tools to live with and treat conditions such as:

  • Childhood asthma
  • Diabetes
  • Congestive Heart Failure
  • High Blood Pressure
  • Cardiovascular Disease

Inpatient & Outpatient Hospital Services

  • Diagnostic testing
  • Laboratory work and X-rays
  • Prescription drugs
  • Other medically necessary services during a hospital stay

Home Health Care

  • Durable Medical Equipment
  • Incontinence supplies
  • Medical supplies, equipment, and devices

Emergency Services

  • Ambulance transportation
  • Hospitalization
  • Screenings, as medically necessary

Maternity Care

  • Prenatal care
  • Delivery
  • Midwife services

Vision Care

Including eye glasses and yearly exams.

Dental Care

Preventive dental services, yearly exams and cleanings.


Care Select Health Plans

When choosing a Care Select health care provider, you have two choices: MDwise and Advantage. This is where you will find your Primary Member Physician (PMP) who will lead you through the course of your medical care and provide referrals to other doctors and facilities. If you ever have questions regarding your coverage, the first place to go is calling your health plan. You may also speak with enrollment broker, Maximus for benefit questions.
