Predictable Cost Plans

Predictable Cost Plans offer
tried-and-trusted coverage with predictable
copayments, calendar year deductibles and
coinsurance amounts. They are perfect if you
want to know your costs upfront so you can
make informed health care decisions.

BlueOptions Conventional Plans
BlueOptions Conventional Plans offer
comprehensive medical coverage at an
affordable cost. Plans offer well care and
preventative care with no deductibles, and
a comprehensive pharmacy program for brand
name and generic prescription drugs. There
are no referrals required, and you can
maximize your savings by using our
extensive provider network, NetworkBlue.
Other benefits include:
- A variety of deductibles to choose
from
- Low, predictable copayments for
network physician office visits and
fixed copayments on other covered
services from select providers
- Coverage for outpatient therapies
(speech, physical, occupational,
cardiac, massage and spinal therapies

BlueOptions health and dental options -
combined into one affordable plan. They're

cost to you

BlueOptions Temporary Insurance
BlueOptions Temporary Insurance was
designed specifically for individuals
between jobs, just starting out or waiting
for benefits to kick-in with a new
employer. Temporary Insurance coverage
offers major medical benefits in
increments of 30, 60, 90 or 180 days, with
an option to renew up to a maximum of 180
days per 12-month period. This plan
includes coverage for a full range of
hospital and physician services with no
referrals, and you can choose any provider
or facility, in-network (NetworkBlue) or
out-of network, with more cost savings
when you use network providers. Other
benefits include:
- Tiered options for NetworkBlue
hospitals for added cost savings and
predictability
- Pharmacy program with discounts on
all generic and brand name
prescription drugs at participating
pharmacies
Plus, this policy is available without
medical underwriting and is considered
Creditable Coverage for when you are ready
for long-term health coverage from BCBSF.
HSA - Compatible Plans

Looking for a less taxing route? Try one of
our high-deductible plans that are compatible
with tax-deductible Health Savings Accounts
(HSA). BlueOptions HSA-Compatible Plans allow
you to use tax-free contributions to help
cover out-of-pocket health care expenses,
including qualified deductibles, copayments
and coinsurance. Plans offer lower premiums
and more cost sharing for covered services
because of the higher deductibles that are
covered by HSA contributions. HSA accounts are
portable, so you can take it with you if you
change jobs. Other benefits include:
- Choice of 0% to 20% coinsurance after
calendar year deductible is satisfied
- Pharmacy program choice of low
copayment for generic prescription drugs
or discount program for virtually all
prescription drugs
- Available child only coverage for
children ages one to 17
HSAs offer three ways to save: tax
deductions on contributions; tax-free interest
and investment gains; and tax-free withdrawals
for qualified medical expenses. It's a smart
way to save on health insurance costs.
Lower Cost Plans

BlueOptions Lower Cost Plans are lower
cost, but not lower value. Plans offer
comprehensive medical coverage with lower
premiums than conventional plans, made
possible with higher out-of-pocket limits and
cost-sharing features. Coverage includes well
care and preventative care with no deductibles
and, as with all BlueOptions plans, there are
no referrals required - even for specialist
physicians. Plans include predictable
copayments for covered services and options
for catastrophic coverage for hospitalization,
surgeries and related services. A
comprehensive pharmacy program for brand name
and generic prescription drugs is also
available. Other benefits include:
- 50% coinsurance on covered services
for in- and out-of-network services
- Low copayments for network physician
office visits
- Coverage for outpatient therapies
(speech, physical, occupational, cardiac,
massage and spinal therapies included)
Plus, on some plans you can add maternity
benefits, and child only coverage is available
for children aged one to 17.
Limited Benefit Plans

These lower-priced plans are great if you
appreciate the value of medical coverage, but
don't want to pay for benefits you might not
use. They provide peace-of-mind with coverage
that's there when it's needed most.
- Plans with Hospital and
Surgical Coverage
BlueOptions Hospital and Surgical Coverage
is a Limited Benefit Plan that offers
"catastrophic" coverage for major
illnesses and emergencies requiring
hospitalization or surgery. It's perfect
for healthy individuals and families who
don't visit the doctor often or take many
prescription drugs, but want benefits for
inpatient and outpatient hospital charges
and physician services for covered
hospital admissions. Well-child care
physician visits are included, as well as
diagnostic services related to surgery
(X-rays, ultrasounds and CAT scans).
HSA-Compatible plans are also available
under this plan. Other benefits include:
- Emergency room trips, even if you
are not admitted to the hospital or if
no surgical services were performed
- Pharmacy program with discounts on
generic prescription drugs
- Child only coverage for children
ages one to 17
And, as with all BlueOptions health
plans, you can choose an in-network (NetworkBlue)
or out-of-network provider or facility,
with greater savings if you stay within
the network. The choice is yours.
- BlueOptions Essentials -
Routine Care
BlueOptions Essentials - Routine Care is a
Limited Benefit Plan that offers basic -
but very important - coverage for routine
medical needs. This plan covers a certain
number of physician visits, per covered
member. You can also take advantage of
adult and child wellness visits, and other
services up to a benefit maximum of
$25,000 per person, per year. You can see
any provider without requiring a referral.
Other benefits include:
- Six physician office
visits/services - per person, per
calendar year
- Substance abuse therapy
- Pharmacy program with discounts on
generic prescription drugs
Plus, you can add maternity benefits,
and child only coverage is available for
children aged one to 17.
